Diagnostic laparoscopy as the definitive means of hernia detection? - by: Chaunce123

For the difficult to detect hernia cases that are harder to pin down, but could exist and could be problematic, is diagnostic laparoscopy the definitive method of determining whether a hernia exists or not?

To simplify it greatly, it is as obvious as sticking a camera in the belly and looking around for an obvious hole somewhere? Find something and fix it, find nothing and no harm done? Is there much risk to that approach? Are some patients better suited to this than others?

I have seen what sounds like variations of this discussed on this forum, it sounds logical but is it just too invasive to be a standard diagnostic method for hernias?

Hi, I am a 62 yr old woman and I have been having episodes of severe groin pain on the left side radiating down my left leg to the knee. Sometimes the pain radiates on the left side of my lower back to my waist. I have had these episodes on and off mostly lasting only about 90 minutes. I have chalked it up to gas pains in the past. Last week I had an attack and wound up in the ER in severe pain, BP high, nausea. They did a CT scan with and without contrast and nothing showed up except a cyst on my pancreas. (I will be having 2 MRI's done in a few weeks for this). My main question is; is there a better way to see and/or diagnose a femoral hernia? If it is, in fact, a hernia would the pain come and go? This last episode the pain lasted 7 hours and then went away. Right now I am having spasms in my lower back, waist level, that seems to be warning me to take it easy. Not sure if this has to do with anything. Oh and for the past 4 months I have been having pain in my groin, both sides, when I am laying I my side with one leg on top of the other I cannot lift the top leg without having that pain. I also get that same pain in the groin when getting in and out of the car. I don't know what to ask of my doctor but I fear the next attack coming. Please help.

I posted before.

Just finished major abdominal, hip and pelvic muscles repair for tears, distasis, muscles were reattached to the bone. I finally have proof of what I was saying:)

There's a few last problems I feel.

1. Hiatal hernia: with diaphragm tear. I can only prove hiatal hernia but I can feel the tear in diaphragm. Is there anyone on the east coast (I'm in upstate ny) that would fix a hiatal hernia AND repair any muscle tears around the hernia? I have the hernia due to diaphragm trauma.

2. Levator ani: I'm pretty sure my levator ani has ripped from the bone. I can feel it very clearly from my pubic bone to my rectum. When I walk it feels like my pelvic floor is going to rip. Is there anyone who would fix this? Ob surgeon? There probably weakness/tearing around my vaginal wall as well.

These are all part of the trauma I suffered from giving birth 4 years ago.

41 years old: 107 lbs.

Use of Opiates for long term chronic pain - by: kdkae1

Dr. Towfigh,

You are completely right about opiates. I have been seeing a pain management doctor for over 2 years for chronic groin pain. After several RFA's procedures, will little success, he put me on a Butrans patch weekly and vicodin prn. The Butrans helped alot to begin with, but I was gradually increased from 5mcg to 20mcg weekly. Once what worked so well only gives me minimal pain relief and I now use Vicodin at least twice daily. I need it more but I can't work and function properly when I take it. Dr. Chen redid my right inguinal hernia in April and found a recurrent inguinal hernia and an open hernia in the abdominal wall. Just recently he redid my left side and found a recurrent inguinal hernia and a "new" femoral hernia. I feel much better than I have in a long time but still have residual pain on the right side. I noticed in April when prescribed percocet and ibuprofen that once I stopped percocet 1 week later, that the ibuprofen provided me more relief than the opiates had in the past. Opiates help some for post op pain, but not well for me as I had developed a tolerance to them. Chronic opiate medication should never be used long term as the side effects are many, to include constipation, which can cause future hernias. I have found that ibuprofen at higher doses work better. I noticed in one of your articles that you use a combination of tylenol, naprosyn, and homeopathic medications. Could you tell me what you use for chronic pain other than opiates. Thank you.

New guy here.

I'm a 69 year old Masters Division powerliter. I just competed on Saturday the 30th of July, and when I woke up on Sunday morning, I noticed a small bulge about a half inch below my belly button. I discovered this forum after doing a couple days of internet research, and I assume that I have sprung an umbilical hernia. My bulge is about 3/4 of an inch in diameter, or less. It isn't noticeable while standing, or lying down. It only presents when I apply abdominal pressure, e. g., valsalva breathing, and can be retracted with a push of the finger.

My questions:

Are "below the navel" hernias more uncommon than hernias that present in the navel, or above the navel?

Is the prognosis for an umbilical hernia dependent upon the precise location?

Does this type of hernia always advance in size? I have been lifting/training seriously for a number of years, and have well developed abdominal muscles & core strength.

I am currently visiting in California, and will be making an appointment with my VA physician upon my return to New Mexico next week.

I appreciate your assistance.

I need to get in touch with other people that have had the amount of surgeries that I have had and within two years. Here is my story. It began with a appendix surgery. During the appendix surgery the doctor discovered my Gallbladder was riddled with cancer so it was removed. Then they discovered there was cancer in the liver so some of the liver was removed. Then I experienced two infection surgeries. Then they dis covered I had a hole in my abdominal wall and 2 hernias protruding from it. I then had reconstruction surgery to my abdominal wall and the hernia's were repaired. All this started July of 2013-2015. It's been a year since all the surgeries and my question is:
1. Is it normal to be in so much pain now
2. Has anyone experienced anything comparable to what I have been through so I can talk with them and compare what it feels like inside of my stomach and compare pain.
3. My age is 57, I'm going through menopause the last two years along with this and I am banging my head against the wall on trying to loose weight. I am now finally active, I walk, pick garden produce, can, get groceries, cook meals, laundry , mow the yard and still no weight loss and still so much pain in my abdominal area it hurts very bad.
4. My surgeon says that after the amount of surgeries that I have experienced and such a short time span that its normal to hurt "LIKE HELL" is exactly how he put it.
5. Is there anyone out there that can relate to having a similar situation that I can talk to so I can see if my pain is normal and compare how yours is, then maybe I will feel normal? Because I don't. Surgeons, Doctors, Nurses, Anyone please only reply if you really have info. or have had a similar situation.

Chronic Pain After Inguinal Hernia Repair (Boston) - by: Michael

Hi Dr. Towfigh and Dr. Earle. My name is Michael and I live in Massachusetts. I'm 5'6" and weigh about 140 pounds. I had Open Right Inguinal Hernia Surgery at the Mass General Hospital in Boston on October 16th, 2015, and I have been in pain ever since.

I have been to so many doctors here in Boston the last 10 months, and I have gotten no where with them. More then half I don't think believe me. I just know something it wrong. I have been trying my best to find out what is wrong and if I can be helped in any way, or if I just have to live with the pain for the rest of my life.

It took me this long to find you Dr. Towfigh. I called your office twice last week, since I decided to fly out there to see you. Obviously having a possible solution to the pain easing up would be a dream come true, but at this point......I would just be really happy to finally know what is causing it.

I asked a lot of questions, and learned that you would probably order some scans. I was pretty surprised because pretty much almost every time I've seen a doctor here, I have asked "Is there any sort of scan that might show something, anything at all?" And they have all said no. I asked is there any way to get an idea of what scan(s) you would possibly order. For the main reason my insurance would only over 80% of the costs for them out there, and they would be free here. Plus already having them ready for you would be more convenient for us both I would think. I learned about this forum and got really happy to learn that you are nice enough to check it when you have time. I got even more happy when I actually noticed that there is a doctor that is in Massachusetts, Dr Earle checks this forum sometimes too. I am not sure if he treats/sees patients for pain after Hernia surgery's or not though, (If not, do you know of anyone in Massachusetts that does?) I am actually planning on calling his office tomorrow and asking them, and if so, making an appointment with him first to see if he has any idea what is going on with me. Maybe he also would know some possible scans to order? I don't know. I know it probably would be hard for you to even say what scans might show something without seeing me in person. I have only had one type of scan, and it was a Ultrasound of my scrotum, when I went to the Emergency room back in Jan because it was so bad. They thought it was a blood flow problem. (Test came out normal)

I am going to post a Facts/List of my symptoms and pain. Followed by some of my story after. Which I'll kind of start to ramble and most of it probably won't help you figure out what is wrong with me. But I don't know what else to do. Ever since this has started, I remember my mother used to sometimes say (she suffered from chronic pain) "When people are in pain, they don't know what to do"

Facts/Symptoms:

Open Right Inguinal Hernia Repair Surgery on 10/16/15 with a Johnson & Johnson Proline Hernia System - Incision 2 inches (The op report says they thought it was a direct hernia, but it ended up being a indirect one)

Severe pain at the incision site, starting as soon as I woke up. (A "Soreness" type of pain) I really couldn't do anything for the first 3-4 months. Going to the supermarket was a big deal for me.

Starting from the top of the incision about 1/3rd down (little over 1/2 inch) You can see and feel the Mesh Screen sticking out of me, and this is the main spot for the incision pain but I can feel it above.

3 weeks after surgery, my right testicle started to hurt "Soreness" pain, all of the time. and also started to get larger. Also at random times during the day, get a "shooting" pain from the incision, down to where my testicle is.

Have seen many doctors.Out of them, 2 different Urologist say my spermatic chord is "slightly" inflamed. And also my epididymis is 4-5 millimeters larger (I thought and feel like it is some sort of lump, but they said no) and in this enlarged area of the epididymisi s sore to to touch.

My right pelvic bone and hip sometimes gets a feeling like a "banged" feeling. Like if I ran as fast as I could toward a brick wall and slammed the bone into it...is the best way I can describe it.

Both where the screen is, and testicle always hurt, but things that make them worse (besides touching them, which does) that I have noticed that can't be a coincidence are:

Testicle: If I get excited (erection) and it goes away right away by itself 1-2 minutes later, and about 10-15 minutes after that the testicle pain will get a lot worse. And if I were to ejaculate, it will get A LOT worse then if I did not.

Incision/Screen: If I eat (not even overeat, but the more the worse) It starts bothering me a lot more, especially when I am sitting up, in a driving type of position.

Things that make it better:

Testicle: Nothing makes it better that I have discovered so far

Incision: Lidocaine ointment actually helps a little bit. and I have had 1 shot of lidocaine and that helped a bit too. But didn't help with the testicle.

Weeks/months would go by and I'd start to feel a little better. But all of a sudden it is like someone hits a "rewind" button, and im back how I felt months ago. Overall I am definitely a lot better then I was. It started calming down in the begining of June I am lucky enough to say. But from October to May were a so awful. I still have pain just not as bad as then . Im afraid something is wrong with the mesh that will cause me problems later on down the road. I wish there was a way to know. But then:

NEW PAINS THAT JUST STARTED 1 MONTH AGO!!!::::::

About a month ago, this new pains started that I have never had before. It is a "Burning" pain. It feels like I'm on fire. And its not just where the incision is. Its on the side of my thigh, lower back, butt, front of my leg. It doesn't go down too too far though. Maybe like 1 foot or so, but its all around.

I have no idea if this is nerve pain, something is wrong with my spermatic cord, something is wrong with the mesh (I feel like it is just ....in wrong, but how could i know :( (maybe all of the above) This whole epididymis thing is weird. I have been reading for hours about peoples problems and no one has mentioned a swollen/enlarged epididymis that is sore and hurts to the touch.(I swear it gets bigger when the pain starts after I get excited)

Sorry I am making this way too long. A wise man once told me, "Doctors just want to hear the facts!, dont start telling them a story, or you'll start to lose them" I don't want to lose you!!! Ahhh

So from what I explained, do you think any sort of scan would show anything that is wrong? Also I read that scans can shrink the mesh itself, and my mesh is the kind that can shrink from the heat? I don't know...I'm sure you do.

I read something recently on another forum about this Ultrasound, which I'll quote:

"The test was called a “Musculo-skeletal ultrasound” which was not of the testicles but focused on the landmark anatomy of the inguinal ligament, femoral canal, inguinal canal with content of spermatic cord or round ligament, inferior epigastric vessels, conjoint tendon, deep and superficial inguinal rings. Under billing, it shows as Code # 76705 and is called “Ultrasound of Abdomen” for some reason. However, I would make sure that your physician uses the terminology “Musculo-skeletal ultrasound of the inguinal area with Valsalva.” This will ensure that even a retracted hernia will be discovered due to the straining and the way that makes a hernia “pop out” or show other issues. In your case it would help at revealing any recurrence."

Or is that only good for detecting a Hernia?

I'll start some of the long story, and I will being a lot of details out (Yes I am leaving out details and it is still this long) If you have any questions about anything please ask. I might of left something out that I didn't think was important, but it would be important to you to know.

Around June 2015, I started getting "a strange feeling" on the right side of my groin and my right testicle. It was just that, it wasn't even a painful feeling at all. When I felt it, was more like "What the heck is that" At first it didn't happen often at all. A few weeks went by and it started happening 2-3 times a day so I figured it was time to make a appointment with my Primary Care Doctor.

He examined me and didn't catch it at first (I didn't know what he was checking for at a time was a Hernia) On the 2nd or 3rd attempt he said "I think you might have a Hernia" if you do it is very small. I had heard of them obviously, but I didn't know much about them at all. He referred me to a General Surgeon at the Mass General Hospital in Boston.

Did a little research about Inguinal Hernia's before the appointment with the General Surgeon. Learned that people mainly get them from heavy lifting. At this time i was pretty surprised because I don't and haven't done not just any heavy lifting, not much lifting in general at all for my whole life. I was at that time and for about 12 years before a pretty active jogger though. Jogged 5-6 miles almost every day for years, so thought maybe that is how. I really don't know and I guess it doesn't matter now.

A month later I had my appointment with the General Surgeon. After a quick exam he said yup you have a Hernia. He went on to explain that it was a "easy" "routine" surgery and I should be completely healed in 2-3 weeks or so. I asked the typical questions like "What happens if I don't get surgery now" He said it'll just get worse and worse so it is better to do it now. I have had a couple of surgery's before and since he made it sound like something so casual and like it wasn't a big deal at all. I said sure lets do it, and set up an appointment for October 16th 2015. Little did I know at the time that was the day that was going to change my life.

Day of the Surgery:

Was calm as anyone could be. Since in my mind this was a easy/routine thing and from what I was told it doesn't even hurt that much.
I woke up after surgery in the most pain I have ever been in my life. I was pretty surprised and I remember asking over and over "Is it supposed to hurt this much?" I really did feel like something was wrong...But...How would I know? It's not like I've had Hernia surgery before, so what can I compare it to? All I know is that it hurt a lot more then I thought it was going to, and the chair they wanted me to sit in I couldn't because it hurt so much. They gave me some pain meds and my friend took me home shortly after.

4-5 days later I called since I was in so much pain. I wasn't sure if I was just being a wuss, or something was wrong. All I know it was unbearable where the incision was. I couldn't even wear pants. Even something brushing up against/touching it in anyway at all made it hurt so much more. So the surgeons nurse called me back, and told me to take the bandage off. This is the first time I saw it. The incision is only 2 inches long, but starting at the top going down about 1/3rd on the length of the incision, it was sticking out and extremely swollen for that first 1/3rd or so. (And still sticks out to this day, I thought it was just extra swollen at that spot, I didn't know why, I thought to myself "Maybe he cut too deep there by mistake because I'm Skinny?" But I later learned it was the mesh itself that was/is sticking out) And that part that was sticking out was the main source of my pain. I thought something was wrong for it to look like that in that one area compared to the rest, so I asked if I could come in to my follow up appointment earlier then I was supposed to.

Surgeons says oh its supposed to look like that and started to rub it really hard. "Rub it, it'll go away it's good for it!" I remember screaming when he started doing it. It caught me really off guard.

All of a sudden, about 3 weeks or so after the surgery my right testicle started hurting. (And still hurts to this day) I thought something else was wrong. So I called and went in again. He said, "Oh that's normal it'll go away in a month" I was happy and said oh ok.

It didn't go away, in fact I went to the Emergency room in Jan (3 months or so after the surgery) because the pain was so bad, I thought something was wrong. They thought it was a blood flow problem so they did a ultrasound of my scrotum but everything was normal.

Ok there is obviously a lot more doctor visits and pain but I have decided at the rate I'm going this will be a short book and I have made this post long enough so....fast forward to now.: My testicle still hurts, my leg/back/thigh etc feel like its on fire at times, and I can feel (feel as in not only by touching it, i can just feel it...inside of me, all of the time w/o touching it) this mesh sticking out of me, it just seems so wrong.

Two years after Surgery - Thanks to Dr. Goodyear - by: WasInTN

Greetings
July 31st marks the end of 2 years since I went down the skillful knife of Dr. G for my right side IH. As most of you might know I am doing good and kicking well.

I have no pain of any sort and doing everything. But due to my fears I still do not lift heavy objects since lifting may cause IH on left side. So life is going on. I walk daily, am a vegetarian, and when time allows, I take a jog or do window shopping in a mall. I had my physical and everything looks good. PCP also recommended I go for a Colonoscopy and a stress echo test for heart since my last stress test was 10+ years ago and never had Colon checked.

Colonoscopy is negative and is well. All blood tests are negative too. And stress test is negative. All organs good. There is one caveat. Since the IH is not causing me any trouble, I started eating too much and gained a bit of weight. Before the surgery I had pain if I ate more than one plate but now that pain is absent due to surgical repair; so I am eating too much. I am now close to 150 (used to be 145 plus or minus). Need to watch what I eat. huh? As before I continue to use one Vit B and one Niacin capsule almost daily. This Niacin helped control my TGL (were 230+ and after Niacin they came down to 90-120). If it matters to anyone, two days before surgery I had to drive from Chicago to Kentucky and IH caused me jerks while I drove. But two weeks ago we drove from KY to Niagara and came back with a round trip of 1800 miles and I had no pain; nothing.

The other "concern." The surgical site still has some hard area on surface which I would say is like one inch. It feels hard when I touch it but no pain. It also itches occasionally. Sometimes the itch needs a comb to scratch [usually in bathroom.] But outside in office or elsewhere no itch, nothing. As Dr G said and Dr. Towfigh confirmed, the healing from surgical site takes several weeks to months and years. It is scratchy slow. :-) I am not worried a bit and let go.

Hi Dr. Towfigh,

I am a 28 year old male who has had epigastric hernia surgery repaired via primary repair in 2013. I then had that repair fail and had the incisional epigastric hernia repaired this past April. Looks like it has failed again. Both repairs were at the Shouldice clinic. Neither repair involved mesh. Looking into a good Hernia surgeon in Michigan as I will not be going back to Shouldice. Wondering if you can recommend anyone? I am fit, don't smoke, no trouble going to the bathroom so I assume I am genetically predisposed. I figure I will need mesh this time. Any guidance appreciated. Thank you.

parietex progrip removal, an exercise in futility? - by: marcello71

I had bi-lateral inguinal + umbilical hernia surgery with 3 pieces of covidien mesh(2 pieces of parietex progrip in groin & 1 piece of symbotex composite in umbilical) implanted in Feb 2015. It all began from me lifting things for my mother in Oct 2014 when I became over exerted feeling like things had sunk downward a bit in my stomache which followed with almost 10 days of constipation. After a ct scan at the ER showed only 1 very small umbilical defect containing fat, the ER doc came in saying "the radiologist only sees one hernia in there but I felt an inguinal hernia too, its there trust me" so I was sent to a surgeon. The first I went to said, "you don't have any hernias, come back if something pops out". So I went for a second opinion with that surgeon saying, "you have four hernias and I can fix them for you but I need to use mesh". So being frightened of one of these hernias popping out into my scrotum costing me a testicle, I had the surgery which he did laparoscopically tapp. after the surgery I said to him, "so you used 3 pieces of mesh, those hernias were bad huh? I mean it was necessary then huh?" to which he replied "well it would of been necessary in the next 3-5 yrs probably, it was a preventative procedure" I was shocked. Since the surgery I have had all sorts of complications from allergic/autoimmune(bloating, swelling, inflammation, tremors/shaking, etc.) to groin/testicular pain(between 2-8 out of 10 at times) as well as vascular issues with my genitals turning purple at times of pain & activity. This prompted me to get other opinions with numerous surgeons since the surgery saying, "I don't see why he did the surgery in the first place, you had no real hernias". Finding out it was completely unnecessary has added definite psychological trauma, missery & depression to my situation especially since I was a completely healthy & fit(6'1", 190 lb) 29 yr old guy before this surgery. So finally I am here to ask what the optimal timeframe is to attempt to remove this parietex progrip & is it even possible without me losing a testicle? Because of the Polylactic acid microgrips that take 18 months to dissolve, is it better to attempt removal sooner to prevent more tissue in-growth or later once the microgrips have dissolved? I also believe I may have a solution or idea to dissolving them quicker or de-laminating it easier so to speak from all the vital structures in the pre-peritoneal space(triangle of doom: epigastric/iliac vessels, triangle of pain: sperm cord/ilioinguinal & other nerves) and wish to have a surgeon confirm or deny its feasibility...

I am looking for hernia surgeons on the west coast who do no-mesh inguinal repairs on a regular basis, enough so they that have confidence in the procedure with good outcomes.

Does anyone have any leads for this? Any and all west coast locations are fine. I am Washington based but will travel as necessary. Thank you in advance.

I will try to make this short. I am a 37 year old man. I had pain in the front and back of my left hip during exercise that gradually came on 4 years ago. It kept getting worse to where I would get aching on both sides of the hip after sitting for more than an hour or so. Leaning forward made it even worse. Standing for more than 30 minutes or so would cause aching. Running/hiking for more than 15 minutes would hurt so I had to stop doing that. A general surgeon said he thought I had a small, inguinal hernia which he could repair and "see if that worked". An MRI showed something that could have been small, bilateral fat filled hernias, but ultrasound showed nothing. I had an MRI of my hip joint that showed a small labral tear, so I had labrum surgery. That did not work. I had a second labrum surgery (releasing psoas tendon that time), also did not work. Now I still have pain and weaker hip flexion, though the ortho surgeon says my labrum looks fairly good.

I just went to another general surgeon a few weeks ago who palpated and said I had a small hernia on the left, and possibly the right too. I got a repeat MRI which the radiologist said was unremarkable, no hernia. No other significant findings. So, I don't know if I really have a hernia or not. I'm not sure what to do now, I am in constant pain and it is greatly affecting my life in a negative way. Should I proceed with a hernia surgery and hope for the best? I also still have pain in the back of the hip, which is probably not related to the possible hernia. It might be bursitis/tendonosis, and is a separate issue. I'm also starting to feel an occasional twinge in the front of the right hip as well. Would that make sense I'm starting to get a right hip/hernia problem now if I really did have a small hernia? I"m getting a third opinion from another surgeon next week. I don't know what to do, I can't even walk a mile or even sit in a chair without pain. I've tried PT, acupuncture, chiropractor, labrum surgery, etc. to no avail. I used to be very athletic, ran a half marathon, played basketball, did boot camp workouts, etc. Now all I can pretty much do is yoga.

I need to get in touch with other people that have had the amount of surgeries that I have had and within two years. Here is my story. It began with a appendix surgery. During the appendix surgery the doctor discovered my Gallbladder was riddled with cancer so it was removed. Then they discovered there was cancer in the liver so some of the liver was removed. Then I experienced two infection surgeries. Then they dis covered I had a hole in my abdominal wall and 2 hernias protruding from it. I then had reconstruction surgery to my abdominal wall and the hernia's were repaired. All this started July of 2013-2015. It's been a year since all the surgeries and my question is:
1. Is it normal to be in so much pain now
2. Has anyone experienced anything comparable to what I have been through so I can talk with them and compare what it feels like inside of my stomach and compare pain.
3. My age is 57, I'm going through menopause the last two years along with this and I am banging my head against the wall on trying to loose weight. I am now finally active, I walk, pick garden produce, can, get groceries, cook meals, laundry , mow the yard and still no weight loss and still so much pain in my abdominal area it hurts very bad.
4. My surgeon says that after the amount of surgeries that I have experienced and such a short time span that its normal to hurt "LIKE HELL" is exactly how he put it.
5. Is there anyone out there that can relate to having a similar situation that I can talk to so I can see if my pain is normal and compare how yours is, then maybe I will feel normal? Because I don't. Surgeons, Doctors, Nurses, Anyone please only reply if you really have info. or have had a similar situation.
